Spring Tour 2018 Passport and Notarized Parent Form Information

 

  • Your passport must still be valid 6 months after returning from our trip (October 1, 2018) and have at least 2 blank pages
  • Passport cards will NOT be acceptable for this tour
  • We will need 2 colored copies of the photo page of your passport and 2 colored copies of any visa or permanent resident card, if applicable. If you have a valid non-expired passport, we appreciate you submit your copies now. There will be a labeled box in the band room. (Final due date for turning in passport copies is Monday, December 11, 2017)
  • Note: Passport new or renewals take 4 to 6 weeks to process. You might want to consider expediting the process.
  • Notarized Parent/Guardian Form – The Parent Permission to Travel Form is for students who will NOT be 18 years old on or before April 13, 2018.  Both parents must sign. If both cannot appear together for notarization, 2 separate notarized forms may be submitted.  We hope to have a notary available at the Winter Concert Thu December 7.  (Final due date for turning in this form is Monday, December 11, 2017)

1) Applicants under 16 years old applying or renewing passport:
No online application or renewal for applicants under 16 years old.
Generally both parents must be present at the appointment.
However there are steps if only one parent can appear.

2) Applicants age 16 or 17 years old applying or renewing passport:
No online renewal for applicants 16 or 17 years old.
One parent should be present with child at appointment.
